Understanding Gum Recession

What causes receding gums, and why does it matter?

Gum recession is the process in which the gum tissue surrounding a tooth wears away, exposing the underlying tooth and bone. When the gum wears away, gaps or pockets develop between teeth that collect food, particles, and bacteria which can cause further infection and further bone and tissue loss.

Top causes of receding gums:

  • Gum disease: bacteria and periodontitis destroy the bone around teeth
  • Clenching or grinding teeth at night (bruxism), which most patients do not realize they are doing
  • Overly aggressive toothbrushing and other trauma
  • Previous orthodontic treatment
  • Genetics and the anatomy of your mouth
  • Smoking and vaping, which damage gum tissue

Unfortunately, receding gums do not naturally grow back and there are no natural remedies to regrow gums. Once the gums have been damaged to a certain point, they will no longer rejuvenate without medical intervention.

The Chao Pinhole Technique

Cover exposed roots without grafting, scalpel, or stitches

This gum graft alternative (gum rejuvenation or "lunch-time gum lift") is performed through a tiny pinhole made in the gum well above the teeth. Specialized instruments gently loosen and gradually reposition the gums back to a healthier, more beautiful, and youthful position. Dr. Ressler places soft pieces of resorbable collagen (that gradually melt away) to support and stabilize the new gum line, without using gum grafts.

  • No scalpel incisions and no stitches
  • No gum taken from the roof of the mouth
  • Done with local anesthetic, takes about an hour
  • Multiple teeth treated in only one appointment
  • Works for upper and lower teeth
  • The pinhole usually disappears completely within 24 to 48 hours
  • Instantly recognizable results

Published in the International Journal of Periodontics and Restorative Dentistry in 2012, the technique was found effective, predictable, minimally-invasive, and with an exceptionally high rate of patient satisfaction. Not every patient is a good candidate; a consultation is the best way to find out.

Dr. Ressler used the Chao Pinhole technique to correct Julia's receding gums without the traditional gum grafting surgery. Before and after: 6 months post-op photos.

Julia, a Ressler Dental patient treated with the Chao Pinhole technique Julia's gum recession before and after Chao Pinhole treatment, 6 months post-op
"I noticed my gums receding. I didn't like the areas of recession because aside from being painful it made my teeth look 'long'!! I was embarrassed, and I did not feel comfortable smiling. I first heard about this procedure from Dr. Ressler who is a gum specialist and I was very excited to get my issue fixed without the pain of the standard surgery - I did not know there was another option! I was completely numb during the procedure and I felt no pain. I drove myself home. I had no concern and no discomfort, and I was back to work the next morning. When I look in the mirror, I don't see the unpleasant dark exposed root - I see my smile the way it used to be and that makes me very happy! My gums and my smile look amazing - I feel confident smiling again - I would absolutely do the Chao procedure with Dr. Ressler again. Dr. Ressler is very kind and he truly cares about his patients . . ."

Questions

Receding gums questions, answered

What causes receding gums?
Receding gums or 'gum recession' describes the loss of gum tissue exposing the roots of teeth. Bacteria and periodontal disease may cause gum recession as the underlying bone is destroyed around teeth. Other causes of receding gums include orthodontic treatment, involuntary clenching or grinding of teeth during sleep (bruxism), or less commonly, overzealous toothbrushing.
Can receding gums be reversed or repaired?
Receding gums can often be stabilized, and in some cases improved, if you visit a gum specialist to determine the cause of your recession and get appropriate treatment. Unfortunately, there is no effective home remedy or self-cure for receding gums.
What is the Chao Pinhole technique?
The Chao pinhole method uses surgical instruments placed through tiny holes made under your gumline to loosen your existing gum tissue to reposition it over the roots of teeth with gum recession. This technique is favored by many patients over traditional gum grafting who often consider it less invasive, because there is no scalpel cutting incision needed or multiple stitches used with other techniques. Although it can work very well and provide dramatic results, not every patient is a good candidate for this method.
What is gum grafting?
Gum grafting is a dental procedure performed by a gum specialist to improve the quantity and/or quality of gum tissue in any part of your mouth. Sometimes gum grafting is used to cover exposed roots to correct gum recession, and other times a periodontist will perform gum grafting to protect the bone surrounding the base of a tooth. Gum tissue can be taken from a donor site in another part of your mouth, or artificial grafting materials can sometimes be used instead.
What is the best treatment for receding gums?
Treatments for receding gums include deep cleaning, scaling and root planing, antibiotics, periodontal surgery, laser gum treatment, nightguards, gum grafting, and the Chao Pinhole technique. Only a gum specialist can diagnose your gum issue and determine which treatment options are right for you.
How do I stop my receding gums from getting worse?
Develop a consistent dental hygiene routine: floss, brush, and rinse every day. See a dental hygienist for cleanings every 6 months, and ask about deep cleaning to stop further gum recession. If you clench or grind your teeth at night, a soft nightguard can ease the pressure; most people do not know that they are clenching at night, but a gum specialist will usually know immediately. If you brush too hard, an electric toothbrush can help. And quit smoking and vaping as soon as possible.
